## Probable cause

THE PILOT'S IMPROPER USE OF FLIGHT CONTROLS DURING THE LANDING ROLL, WHICH RESULTED IN A LOSS OF AIRPLANE CONTROL. FACTORS RELATED TO THE ACCIDENT WERE: THE LOOSE NOSE WHEEL SHIMMY DAMPENER AND THE PILOT'S LACK OF FLIGHT EXPERIENCE IN THIS MAKE AND MODEL OF AIRPLANE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, landing - roll
  - landing gear, main gear strut - vibration (contributing factor)
  - flight controls - improper use of, pilot in command (cause)
  - lack of total experience in type of aircraft, pilot in command (contributing factor)
  - aircraft control - not maintained, pilot in command
- Nose over, landing - roll

## Analysis

THE PILOT STATED THAT THE LANDING WAS NORMAL, BUT THAT A 'VOILENT NOSE WHEEL SHIMMY' OCCURRED AS THE AIRPLANE SLOWED. HE SAID, '[THE AIRPLANE] VEERED SHARPLY TO THE LEFT, [AND] UPENDED.' THE FAA INSPECTORS FOUND THE NOSEWHEEL SHIMMY DAMPENER LOOSE. THE PILOT HAD A TOTAL FLIGHT TIME OF 37 HOURS IN THIS MAKE AND MODEL OF AIRPLANE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910728030369A](https://openairdocs.org/incident/19910728030369A), Jul 28, 1991: Piper PA22150, York, PA, Substantial damage. Nose wheel shimmy on landing rollout. Lost control, cartwheeled. Flipped over. Shimmy damper loose. Incorrect input

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
