## Probable cause

IMPROPER PLANNING/DECISION BY THE PILOT BY ELECTING TO TAKEOFF WITH A KNOWN DEFICIENCY WITH THE EQUIPMENT. THE DEFICIENCY (CRACKED/LEAKING FLOAT) WAS A RELATED FACTOR.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Nose over, takeoff - aborted
  - operation with known deficiencies in equipment - intentional, pilot in command (cause)
  - landing gear, float assembly - cracked (contributing factor)
  - terrain condition - water
  - landing gear, float assembly - leak (contributing factor)

## Analysis

THE PILOT LANDED HIS AMPHIBIOUS PLANE IN A HARBOR. WHILE TAXIING TO THE DOCK, THE PLANE STRUCK A SUBMERGED OBJECT WITH THE LEFT FLOAT. THE PILOT PULLED THE AIRPLANE UPON THE BEACH, CHECKED THE FLOAT AND FOUND A CRACK. HE PUMPED THE FLOAT DRY AND MADE A DECISION TO TAKEOFF. HOWEVER, HE ABORTED THE TAKEOFF, WHEN HE NOTED THE LEFT FLOAT WAS FILLING WITH WATER. SUBSEQUENTLY, THE LEFT FLOAT DIPPED UNDER THE WATER AND THE AIRPLANE NOSED OVER.

- [Accident 19910822036219A](https://openairdocs.org/incident/19910822036219A), Aug 22, 1991: Cessna TU206G, Block Island, RI, Substantial damage. Landed amphibian in harbor. Left float hit object. Taxied to beach. Noted crack. Tried takeoff. Float sank. Upset.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
