## Probable cause

THE PILOT'S FAILURE TO PREFORM AN AIRCRAFT PREFLIGHT, AND REMOVE THE GUST LOCK FROM THE AILERON, RESULTING IN A LOSS OF AIRPLANE CONTROL DURING INITIAL CLIMB.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - in flight, takeoff - initial climb
  - aircraft preflight - not performed, pilot in command (cause)
  - complacency, pilot in command (cause)
  - flight control, gust lock - not removed (cause)
  - aircraft control - not possible, pilot in command (cause)
- In flight collision with terrain/water, descent - uncontrolled
  - object - fence (cause)

## Analysis

THE PILOT SAID HE FORGOT TO TAKE THE GUST LOCK OFF THE RIGHT AILERON. THE AIRPLANE CLIMBED TO ABOUT 5 FEET ABOVE THE GROUND, STARTED TO TURN LEFT, AND STRUCK A FENCE NEAR THE RUNWAY.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911010045609A](https://openairdocs.org/incident/19911010045609A), Oct 10, 1991: Cessna 172N, Toledo, OH, Substantial damage. Took off with right aileron control lock in place. Made left uncontrolled turn into airport fence.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
