## Probable cause

THE FAILURE OF THE PILOT TO REMOVE ALL THE WATER FROM THE FUEL SYSTEM PRIOR TO TAKEOFF WHICH RESULTED IN A POWER LOSS.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - nonmechanical, takeoff - initial climb
  - fuel system - contamination (contributing factor)
  - aircraft preflight - inadequate, pilot in command (cause)
- Forced landing, descent - emergency
- Nose over, landing - roll
  - terrain condition - rough/uneven (contributing factor)

## Analysis

THE PLANE LOST POWER AFTER TAKEOFF AND MADE A FORCED LANDING IN THE ONLY AVAILABLE FIELD. THE AIRPLANE FLIPPED OVER DURING LANDING. THE PILOT SAID HE DRAINED THE SUMPS PRIOR TO TAKEOFF AND REMOVED A SMALL AMOUNT OF WATER. POST ACCIDENT INVESTIGATION DISCLOSED WATER IN THE FUEL TANK, FUEL LINES , CARBURETOR AND ACCELERATION PUMP.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911204059099A](https://openairdocs.org/incident/19911204059099A), Dec 4, 1991: Cessna 182, Westerly, RI, Substantial damage. Lost power on climbout. Flipped over landing. Found water in fuel tanks. Bladder folds in type hold water.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
