## Probable cause

THE PILOT'S IMPROPER PREFLIGHT OF THE HELICOPTER, WHICH RESULTED IN A TIE DOWN STRAP NOT BEING REMOVED PRIOR TO ATTEMPTED FLIGHT, AND THE SUBSEQUENT ROLL OVER AND COLLISION WITH THE RAMP.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Roll over, landing
  - aircraft preflight - improper, pilot in command (cause)
  - tie down - inadvertent use, pilot in command

## Analysis

THE HELICOPTER PILOT ATTEMPTED TO TAKEOFF FROM A PORTABLE PAD. THE HELICOPTER IMMEDIATELY NOSED OVER STRIKING THE RAMP WITH THE MAIN ROTOR BLADES. IT WAS FOUND THAT THE TIE DOWN STRAP WHICH SECURES THE HELICOPTER TO THE PORTABLE PAD HAD NOT BEEN REMOVED DURING THE PREFLIGHT.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920418010409A](https://openairdocs.org/incident/19920418010409A), Apr 18, 1992: Robsin R22BETA, Columbus, OH, Substantial damage. Hurried boarding helicopter due to rain. Neglected to remove tie down strap, nosed down lifting. Rotors hit ground.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
