## Probable cause

THE PILOT'S IMPROPER IFR PROCEDURE IN FAILING TO INTERCEPT THE PROPER GLIDESLOPE PATH.

## Analysis

RADAR DATA SHOWS THAT DURING AN ILS APPROACH TO RWY 5 OF THE COEUR D'ALENE AIRPORT, THE AIRPLANE WAS WELL ABOVE ALTITUDE. THE DESCENT PROFILE FOR THE APPROACH DEPICTS THAT AN AIRPLANE WOULD EXPECT TO CROSS THE OM AT 4,039 FT, AND CROSS THE MM AT 2,535 FT MSL. THE AIRPLANE'S ALTITUDE IN THE VICINITY OF THE VOR, MM, AND AIRPORT WAS BETWEEN 4,900 FT AND 5,100 FT MSL. THE AIRPLANE CONTINUED DESCENDING AFTER PASSING THE AIRPORT AND COLLIDED WITH THE PEAK OF A MOUNTAIN ABOUT 7 MI FROM THE AIRPORT 2 FT BELOW THE SUMMIT ELEVATION OF 3,395 FT MSL.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight collision with terrain/water, approach
  - ifr procedure - improper, pilot in command (cause)
  - proper glidepath - not attained, pilot in command (cause)
  - terrain condition - mountainous/hilly (contributing factor)

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19901121052029A](https://openairdocs.org/incident/19901121052029A), Nov 21, 1990: Beech 9555, Coeur D Alene, ID, Destroyed, 3 fatal. Struck two feet below mountain top on a missed approach.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
