## Probable cause

A MALFUNCTION/FAILURE OF THE HELICOPTER'S MAIN ROTOR LEAD-LAG DAMPER.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Airframe/component/system failure/malfunction, descent
  - rotor system,main rotor hub lead-lag stop/damper - failure,total (cause)
- Loss of control - in flight, landing
- In flight collision with terrain/water, landing

## Analysis

THE ACFT DEVELOPED VIBRATIONS IN FLT WITH SEVERE FEEDBACK THROUGH THE CONTROL SYSTEM TO THE CYCLIC STICK. THE PILOT INITIATED AN EMERGENCY LANDING AND LOST CONTROL JUST BEFORE TOUCHDOWN. THE ACFT IMPACTED THE GROUND WHILE SPINNING AND ROLLED OVER. EVIDENCE INDICATED A MALFUNCTION/FAILURE OF A MAIN ROTOR DAMPER.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19901203052389A](https://openairdocs.org/incident/19901203052389A), Dec 3, 1990: Aerosp SA316B, Vernal, UT, Destroyed. Vibration in cyclic. Elected to land in clearing. Lost rotor RPM. Spun prior impact. Suspect rotor blade dampers.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
