## Probable cause

POOR PREFLIGHT PLANNING/PREPARATION BY THE PILOT IN COMMAND. FACTORS TO THE ACCIDENT WERE: HIGH DENSITY ALTITUDE, EXCESSIVE CLIMB RATE, INADEQUATE REMEDIAL ACTION, AND LACK OF TOTAL FLIGHT EXPEREINCE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - in flight, takeoff - initial climb
  - weather condition - high density altitude (contributing factor)
  - lack of total experience, pilot in command (contributing factor)
  - proper climb rate - excessive, pilot in command (contributing factor)
  - remedial action - inadequate, pilot in command (contributing factor)
  - preflight planning/preparation - poor, pilot in command (cause)
- In flight collision with terrain/water, takeoff - initial climb

## Analysis

AT THE BEGINNING OF A FLIGHT, THE AIRPLANE ROTATED NORMALLY AND ATTAINED AN ALTITUDE OF 30 TO 40 FEET WHEN THE AIRSPEED DROPPED AND THE AIRPLANE SETTLED TO THE RUNWAY. WITNESSES REPORTED THAT THE ANGLE OF ATTACK INCREASED TO 25 DEGREES WHEN THE AIRPLANE LIFTED OFF. BEFORE THE AIRPLANE REACHED THE END OF THE RUNWAY, THE NOSE DROPPED AS DID THE RIGHT WING AND THE AIRPLANE SETTLED TO THE RUNWAY.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
