TCAS MADE CONTACT WHILE FLT GCO7580 WAS DEPARTING PHNL, AT APPROXIMATELY 3800 FT.  FLIGHT CREW INITIATED EVASIVE MANEUVER WHICH RESULTED IN STRUCTURAL DAMAGE TO BOTH LT AND RT ELEVATORS.  (M)
