## Discrepancy

FAILURE TO SECURE JAM NUT ALLOWED ACTUATOR TO UNTHREAD FROM ROD END BEARING.  THIS CAUSED OUT OF RIG CONDITION OF LANDING GEAR AND INABILITY TO EXTEND GEAR TO DOWN AND LOCKED POSITION VIA NORMAL OR EMERGENCY.
