DURING A PHASE 1 INSPECTION,   MAIN ROTOR GRIP HAD BEEN IMPROPERLY ASSEMBLED AND INSTALLED ON THE MAIN ROTOR YOKE CAUSING DAMAGE TO THE NEEDLE BEARING AND RACE.
