DURING A TRAINING MISSION, A FIREFIGHTER BECAME DETACHED FROM THE HOIST HOOK AND FELL TEN METERS SUSTAINING INJURIES. HOIST HOOK HAD NOT BEEN RETROFITTED WITH PRODUCT SAFETY SERVICE BULLETIN.
