FLT 3465 - MIA-MCO - AFTER TAKEOFF AND AFTER GEAR RETRACTION, THE RIGHT MAIN GEAR DOOR FAILED TO CLOSE.  AIRCRAFT TURNED BACK AND LANDED IN MIA WITHOUT INCIDENT.  REMOVED AND REPLACED RIGHT FORWARD GEAR DOOR ACTUATOR.  (W)
