NR 2 ACTUATOR FAILED.  THIS ACTUATOR HAS 1454.9 HOURS SINCE OVERHAUL AND INSTALLATION.  THIRD KNOWN PROBLEM WITH OVERHAULED ACTUATOR 'ALSO SPARE ACTUATOR FAILED'.  OVERHAUL FACILITY NEEDS MORE THOROUGH BENCH TEST OR SEEK OTHER VENDOR.
