## Discrepancy

TAIL ROTOR CONTROLS LOST ACCUMULATOR PRESSURE, FOUND VALVE STEM LOOSE.  SUBMITTER STATED THIS HAS HAPPENED SEVERAL TIMES WITH ALL ACCUMULATORS (INCLUDING M/R) ON 3 DIFFERENT AIRCRAFT.  SUBMITTER RECOMMENDS USING ACCUMULATOR WITH JAM NUT AND SHUT-OFF FOR VALVE STEM.
