## Discrepancy

NR 4 ENGINE, 4TH TURBINE WHEEL LOST HEAD OF RIVET WHICH HOLDS BLADE IN PLACE, CAUSING IMPACT DAMAGE TO 4TH NOZZLE CONTAINMENT RIGHT.  MAINTENANCE REMOVED AND REPLACED 4TH T-WHEEL ASSEMBLY.  (M)
