455 ORD-AZO EXPERIENCED A GEAR DISAGREE WHILE DEPARTING ORD. THE CREW RAN THE QRH AND ELECTED TO RETURN TO ORD. AN EMERGENCY WAS DECLARED.  AIRCRAFT REMOVED FROM SERVICE.
