CLEANERS REPORTED FORWARD GALLEY DOOR COVER LOOSE, AND SLIDE DISLODGED. MAINTENANCE REPLACED FORWARD GALLEY SERVICE DOOR EVACUATION SLIDE IAW AMM 25-62-00-201. CHECKED AND FOUND OK FOR SERVICE.  THE AIRCRAFT WAS GROUNDED.
