AIRCRAFT WAS GROUNDED: FORWARD SERVICE DOOR EMERGENCY SLIDE WAS NOT DISARMED DOOR WAS OPEN, SLIDE FELL OUT BUT FAILED TO DEPLOY.  REMOVE AND REPLACED R1 (FORWARD SERVICE DOOR) DOOR EMERGENCY ESCAPE SLIDE. PER J/C 08L22125001
