(AUS) PILOTS COLLECTIVE LEVER BEARING WORN.  UNAPPROVED REPAIR HAD BEEN CARRIED OUT TO TAKE UP THE PLAY IN THE WORN SHAFT AND BEARING.  UNAPPROVED REPAIR.  PERSONNEL/MAINTENANCE ERROR.
