EMERGENCY DECLARED,  CABIN ALTITUDE WARNING EICAS ABOVE 100 MSL. ACFT AT FL 210, CABIN ALT 11500.  WARNINGS OCCURRED WITH BOTH PACKS RUNNING. THIS ITEM WAS DEFERRED IAW MEL 21-31-1A, VERIFIED MANUAL OPERATION OF OUTFLOW VALVE.
