ENGINE NR 3 SHOWED FAN VIBRATION OF 5.0 UNITS AT LIFT OFF, NO VIBRATION FELT.  PERFORMED VISUAL INSPECTION AND BORESCOPE, DEFECTS FOUND.  REMOVED AND REPLACED ENGINE, IAW AMM 71-00-02/401.
