LEFT CABIN DOOR DISLODGED FROM FULL OPEN, LOCKED POSITION. DURING LEFT BASE TURN TO FINAL, 90 KTS, 1000', LEFT CABIN DOOR DISLODGED BUT WAS STILL ATTACHED. APPROXIMATELY 30 SECONDS LATER, LEFT CABIN DOOR EVACUATED AIRCRAFT COMPLETELY. CREW DECLARED PL. LT UPPER CAR ASSEMBLY (B521M3834966) WAS FOUND TO HAVE BROKEN/SNAPPED.
