FWA - FLT 5722 - EXIT CWP ILLUMINATED ON TAKEOFF WITH LEFT FRONT DOOR INDICATING OPEN.  FLIGHT ATTENDANT REPORTED DOOR WAS CLOSED.  MAINTENANCE INSPECTED AND PERFORMED OPS CHECK OF ALL DOOR WARNING INDICATORS.  ALL SYSTEMS CHECKED GOOD.  (M)
