Installation of Zodiac Intertechnique EROS Crew Oxygen Mask in accordance with West Star Aviation, Inc. Master Document List Report No. 351051MDL, Revision A, dated December 10, 2004, or later FAA approved revisions.

1. Compatibility of this design change with previously approved modifications must be determined by the installer.

FAA approved Airplane Flight Manual Supplement, West Star Aviation Document No. 351050AFMS, Revision IR, dated 12/16/04 or later FAA approved revision is required for Cessna Model 501 and FAA approved Flight Manual Supplement No.

351051AFMS, Revision IR, dated 12/16/04, or later FAA approved revision is required for Cessna Model 551.

Instructions for Continued Airworthiness, West Star Document No. 35105ICA, Revision IR, dated 12/14/04 or later FAA accepted revisions for the Zodiac Intertechnique EROS Crew Oxygen Mask must be maintained as part of the permanent records for each aircraft.

4. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.
