Installation of Wescam nose mounted camera pod 104/12 WESCAM or 116 series WESCAM or Model 12 Series WESCAM in accordance with documents specified in the Configuration Matrix (See Continuation Sheet).

1. The Flight Supplements specified in the Configuration Matrix (See Continuation Sheet) are required.

2. The helicopter must be equipped with high skid gear only.

3. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.

4. Compatibility of this design change with previously approved modifications must be determined by the installer.
