Original Product --Type Certificate N umber :

Installation of Lycoming VO-540-B1B3 engine in accordance with Petroleum Helicopters Inc. Parts List, and drawing PH5000 sheet 1, revised 4/20/64, and sheet 2 and 3 dated 11/7/63 or later FAA approved revision.

1. This modification is eligible on Bell 47G-2A with both skid and float gear.

2. A 47-620-600-25 transmission must be installed.

3. FAA approved RFM Supplement dated 11/19/63 required.

4. The installer must determine whether this design change is compatible with previously approved modifications.

5. If the holder agrees to permit another person to use this certificate to alter a product, the holder must give the other person written evidence of that permission.

This ce rtificate and the supporting data, which is the basis for approval, shall remain in effect until surrendered, suspended, revoked or a termination date is otherwise established by the A dministrator of the F ederal A viation A dministration.
