Description of the Type Design Change: Fabrication and Installation of Kawak Aviation Technologies Model 36100 Sea Snorkel in accordance with Kawak Aviation Technologies Master Drawing List No. 36100-MDL, Revision A, dated October 21, 2005, or later Federal Aviation Administration (FAA) approved revision. This modification must be maintained in accordance with Kawak Aviation Technologies Instructions for Continued Airworthiness (ICA) Manual No. KATI-MM-643, Revision A, November 1, 2005, or later FAA approved revision.

Limitations and Conditions: Approval of this change in type design applies the Helicopter Transport Services, LLC, Model CH-54B, Serial Number 69-18468, only. Rotorcraft modified in accordance with this certificate must be operated in accordance with a FAA approved copy of the Rotorcraft Flight Manual Supplement (RFMS) Kawak Aviation Technologies Systems Document No. KATI-FM-643, FAA approved November 9, 2005, or later FAA approved revision. A copy of this certificate, FAA approved RFMS, and the ICA must be maintained as part of the permanent record of the modified rotorcraft.
