Installation of Survival Kit in accordance with Vancouver Island Helicopters Ltd. Master Document Drawing List V212-0525M- MDL, Rev. 0, dated April 24, 2006. or later Transport Canada approved revision.

Limitations and Conditions:

1. Instructions for Continued Airworthiness documents specified in AML SR02430NY or later Transport Canada accepted revisions, are required with this installation.

2. The Installer must determine whether this design change is compatible with previously approved modifications.

3. If the holder agrees to permit another person to use this certificate to alter a product, the holder must give the other person written evidence of that permission.

This certificate and the supporting data which is the basis for approval shall remain in effect until surrendered, suspended, revoked or a termination date is otherwise established by the Administrator of the Federal Aviation Administration
