From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: during training flight, could not get a green light indicating the nose gear was down and locked after following emergency procedures the aircraft was landed. Nose gear collapsed during landing, damaging both propellers and the nose. No injuries to the pilot or student. VFR conditions prevailed. Flight had originated approximately 30 minutes prior to incident. The aircraft is owned and operated by rochester aviation of rochester, MN. Not a pilot deviation. Nose gear down lock link lower bolt has an 1/16TH inch cotter pin per Piper instructions. Piper service kit 764-378 was installed 200 hours prior to the incident as a method of complying with AD 94-14-14. Lower bolt sheared the recommended cotter pin, allowing the bolt to back out and jam the operation of the nose gear. Submitter recommends a larger cotter pin be employed. M or d report reflecting this recommendation has been submitted.
