From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: 22, January, 1995, Mr. Paul sieli flew in C-182 piloted by Mr. Greg watson and sky dived from altitude of 7500'. Mr. Sieli's main parachute failed to open and reserve was not deployed. Mr. Sieli received fatal injuries when he struck tall oak apartment complex and then fell to the ground. Paramedics arriving at scene removed parachute but failed to note how installed. Parachute (reserve) was taken to flight concepts, a manufacturer and "d" ring deployed. Parachute functioned normal. Flight concepts inspected main parachute and made written statement: parachute pilot canopy was routed through leg strap preventing deployment. Mr. Sieli has a known heart condition which was being investigated by local police department.
