From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: on January 25, 1995, the pilot of N21470 states that he was landing at smi 255 #1, an offshore platform, to pick up a passenger when he heard a noise and felt a vibration. The pilot stopped his approach and hovered over the platform. When the vibration did not stop, he landed the helicopter and completed an emergency shut down. It was noted during inspection of the tail rotor that it had come in contact with wire mesh on the helideck. The pilot notified operations personnel at industrial helicopters and secured the helicopter.
