From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(4) The airman had no airman certificate and was reported to have had only one hour of instruction in the aircraft. (5) FAR should be amended requiring manufacturers of aircraft commonly referred to as ultralights, to inform purchasers that there are regulatory requirements governing the operation of their products. (9) 91.203 in that the aircraft was operated without an airworthiness or registration certificate. 61.3 in that the operator had neither a pilot or a medical certificate. No violation will be pursued, due to the death of the pilot who was also the owner. A safety recommendation was made.
