From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Short field takeoff full flaps. Then dumped same. Settled. Impacted wing low. (-23) N834A, a PA-22, departed runway 27 at Shades State park at approx. 5:00 pm CST. On 10/15/95. The aircraft was rotated at 60 mph and full flaps applied for a short field take off. An immediate left turn was made to stay clear of the trees at the west end of the runway. The flaps were raised after turning and the aircraft began to sink. A left turn again was made in an attempt to exit what the pilot felt was a large down draft. The airspeed was a little low and the aircraft at 100 ft AGL was sinking. The pilot used the carburetor heat and checked both mags. The nose was slightly lowered in an attempt to gain airspeed and the altitude loss increased. The aircraft impacted the ground and rolled inverted. The pilot and 2 passengers sustained minor injuries. The aircraft received substantial damage including the gear, fuselage, wings, flight controls, empennage, engine compartment, and prop.
