From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: victim, monte earl kirkpatrick, made a deliberate parachute jump from BE-50 aircraft, N155P at approximate 12,500 feet. He deployed the main parachute at approximately 4000 feet; however, it did not deploy properly. The victim cut away the main parachute before deploying the auxiliary parachute which also deployed improperly. The victim impacted the ground at a high rate of speed. He was pronounced dead at the scene. According to witness reports, the victim belonged to the Crestview skydiving club, was an experienced military halo jumper, had 2500 free falls, and a total of 18-20 years experience in the sport. Witnesses also stated the victim packed the main parachute a few days prior to the incident, and that the auxiliary parachute was packed by jump shack of deland, FL. Examination of the auxiliary parachute revealed: 1. It was a micro raven 120 9|; manufactured by precisi on aerodynamics, inc. (hwy. 127 north; po box 386; dunlap, TN 37327; phone: 615-949-4688 or 1-800-222-3933); manufacture date July 93; serial number 28321449; maximum speed 130; maximum weight of parachutist 137 pounds. 2. The parachute experienced an unexplained "bag lock" malfunction. 3. The right side tail of the canopy was pulled approximately one foot through a grommet. This caused four lines connected to the right tail to be pulled completely through the grommet. 4. The latest packing date is in question because the date on the parachute packing record, that was found with the parachute, appears to have been changed. The date on the packing record appears to be "29 aug. 95". However, the riggers log and shop log reflect a date of "19 July 95". It was further revealed that the parachute the victim wore belongs to the pilot of N155P which is the aircraft used during the November 25, 1995, jump.

As the FAA filed it
