From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: this is a landing incident. Private pilot deborah falley was being checked out in a tailwheel aircraft with a properly certificated flight instructor in the aircraft. Ms. Falley stated that during the landing rollout the aircraft felt "unstable" and out of reflex she quickly applied the brakes. The flight instructor did not anticipate such a hard brake application. While the instructor verbally attempted to get Ms. Falley off the brakes, the aircraft proceeded to rotate forward until the propeller impacted the ground. The airplane did not roll over on its back and the wings did not touch the ground.
