From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: after departing tulsa (tul), Oklahoma flight #891 was climbing through flight level 260 to flight level 290, the auto fail light on cabin pressurization system illuminated. The pilot placed the pressurization mode selector to stby, which extinguished the light and set 5300 in the cabin altitude window. The cabin altitude started to climb, so the pilot went to manual and attempted to close the outflow valve. Next the altitude warning horn went off, the pilots performed the rapid depressurization checklist and did an emergency descent to 10,000 ft. the pilots completed the auto fail or unscheduled pressurization checklist, checked the flight attendants and passengers, all were ok. The flight proceeded to Kansas City (mci), MO and landed with no further incidents. Southwest airlines maintenance personnel inspected the pressurization system and performed a complete system check in accordance with maintenance manual procedures. All systems operated normally.
