From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: Beech N33BV, operated by west valley flying club, received minor damage when the landing gear collapsed during the takeoff roll. The pilot stated the preflight and runup checks were normal in all respects prior to accepting a takeoff clearance from Palo Alto tower. He stated he had flaps full down prior to accepting a clearance to expedite the takeoff. He stated he raised the flap handle at the same time as he brought the throttles to 17" manifold pressure. He said at approximately 50 knots indicated airspeed, he felt the nose pitch forward, and the aircraft came to an abrupt stop. When questioned about the incident, Mr. Kuhn said he cannot remember with certainty which handle (flap or landing gear) he used "to raise the flaps." He said he had made 10 or 12 takeoffs and landings in the CE-210 the previous weekend. The CE-210 gear and flap handles are placed on the panel in reverse order from the Beech aircraft. The pilot was counseled by sjc FSDO, and stated that he plans to receive additional training in the Beech. No 8020-11 was filed. This completes the investigation and this report is closed.
