From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: the pilot broke out of the clouds while making an instrument let down into palwaukee (pwk), however was unable to see the airport due to hazy conditions. ATC then vectored the pilot back for an ILS approach to runway 16, circle to runway 30R. While circling for runway 30R, the pilot was distracted by the change in sequence of events, the trees on the approach end of runway 30R, and the shortness of the runway. The pilot first heard the landing gear warning horn as the aircraft flared for touchdown. The pilot stated that he did not remember doing the "gumps" check while circling for runway 30R. Airman made a gear up landing following an instrument approach with a circle to land maneuver.
