From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: as flight 6003 entered runway 11R prior to takeoff, the tower informed them that an object resembling a fuel tank filler cap had been spotted near their aircraft on the taxiway. The captain stopped the aircraft and he and the first officer determined that the fuel caps were intact. At liftoff, the tower saw an object fall from the aircraft. It was subsequently determined that it was a wheel. The flight was notified. The captain performed a landing gear inspection, determined that the right main gear inboard wheel was missing, declared an emergency, and returned uneventfully to msp. Right main landing gear inboard wheel fell off aircraft on takeoff.
