From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Ran a wheel off runway on takeoff. Aborted. Tried return onto runway. Nosed over. Beleive tailwheel failed to lock. (-23) during an aborted 100 foot takeoff roll on runway 6, the aircraft inadvertently drifted to the right as the pilot applied engine power. The right main landing gear wheel sunk into the dirt parallel to the runway. Attempting to getting the aircraft back onto the runway by using left rudder and left brake, the aircraft nosed over. As a result, the aircraft's tail section was crushed, the upper wing surface damaged in several locations, and propeller bent and the engine/nose cowl area damaged.
