From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Aircraft took off, smoke coming from aircraft, attempted to return, crashed. (.4)after takeoff, a witness observed a puff of black smoke coming from the engine. Shortly thereafter, the airplane nosed down and descended rapidly. During the on-site examination of the engine, it was observed that the #6 cylinder head was separated from the engine. Metallurgical testing on the cylinder head revealed that the fracture was indicative of high temperature separation or overstress. (-23) after take off, puff of smoke observed coming from engine, aircraft attempted to return to airport & crashed during approach to field.
