From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: the pilot took off from dvi with geu his destination. On base leg to geu RW19, the pilot extended flaps to 15 degrees and lowered the landing gear. Only two (2) green lts. Came on for the main gears. The nose gear green lt did not illuminate. The pilot verified the nose gear position as retracted thru the gear mirror on 4 engine nacelle. This was also confirmed by geu tower personnel. While discussing the problem with the Glendale aviation personnel the pilot actuated the emergency gear release handle without success.. While then attempting to "bump" the nose gear down with the main gears on the runway, the pilot could not add power to prevent the nose of the aircraft from contacting the runway and slid down the runway on the two (2) main gears and nose of the aircraft nose gear still retracted. The props contacted the runway and the aircraft skidded off to the east side of the runway on to the dirt and came to rest tail high. No injuries and no post-crash fire evident.
