From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: pilot deviation of 1/28/98 @ cak. Spoke with Mr. Gene mckenna, he stated that he was acting as PIC of Grumman American AA-S-N74869 on 1/28/98 @ 1420 EST. He and the aircraft owner, Mr. Jeff kostich, a private pilot whose medical had expired, were to fly Mr. Kostich's airplane to wadsworth, Ohio for maintenance to be performed. Mr. Mckenna was unfamiliar with the aircraft and had never flown it or any Grumman aa-s aircraft. As per the ATC tape from Akron-Canton tower, the last reply given by him to the ground controller was "04", the last 2 digits of the altimeter setting 30.04 he was given by the controller to acknowledge the taxi clearance he received. He thought he had received a takeoff clearance and an initial heading of 230 degrees but according to the tape, had not and subsequently departed without a clearance. He stated that he did "know better" but thought he had received clearance. He is forwarding copies of notes he took following the incident so they may be included in the record.
