From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) A Mr. Gus glass (parachutist) was fatally injured when he struck the ground. Witness reported that Mr. Glass grabbed the main chute cord and became entangled. The reserve parachute deployed and also, became entangled with the main chute and never inflated. The lasalle county sheriff's department, noted in their accident report that the main chute was wrapped around the jumper's left arm. The inspection of the parachute revealed no malfunction of the main or reserve chutes. The first time jumper was hooked to a static line and the static line initiated deployment as designed.
