From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-5)privacy data omitted on August 8, 1998, it was discovered the #2 main wheel brake had rotated. The attachment bolts and alignment studs were sheard and the heads of the bolts were popped off. Investigation revealed a different bolt/stud configuration than required by maintenance manual 32-41-41. Evaluation of the brake assembly showed evidence the brake assembly was not properly secured to the flange and wheel assembly. The bolt and stud holes were elongated and had thread marks from the attachment bolts. The wheel assembly had several deep score marks from the brake assembly rotating against it. The main landing gear axle shaft also had several deep score marks caused by the #2 brake rotation. The evidence shows the #2 brake assembly was not properly secured and that it ossilated on the shaft over a period of time until the assembly failed. This investigation is considered closed.
