From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-5)The r/h main landing gear retract/extend actuator failed internally during gear retraction shortly after take off which did not allow the r/h gear to fully retract in the up and locked position. The pilot communicated with personnel on the ground to confrim the unsafe gear indication and tried to ge the gear to go fully down and locked, but was unable get the gear to go over center and locked due to the drag on the landing gear. The pilot elected to land the aircraft with the l/h and nose landing gear in the up and locked position with the r/h landing gear hanging freely. The aircraft received only minor damage and no injuries occured with this incident. This actuator was sent to Mr. Gerard vanaaist manager of the actuator division of eaton, where it was torn down for examination. This tear down revealed that the actuator went into a "freewheel" mode because both of the set screws that hold the no-back spring inplace had backed out. These screws are located in the actuator internally and are installed at the factory at the time of actuator assembly.

As the FAA filed it
