From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Acft was departing lake winnibigoshish when the nose gear collapsed, other circumstances are unknown. (From form 23) aircraft was taking off heading 135 from an ice strip on lake winnipegosis, bowstring county MN at approx 1745 hours. The ice strip is approx. 4000 ft. x 50 ft., wet, slushy with ruts. About 200 feet into take off roll (30 KTS.) The pilot reported that he felt a hard thud in his rudder pedal. The aircraft veered to the left and departed the plowed portion of the frozen lake and into one foot of heavy wet snow. The nose gear collapsed and the prop struck the ice. No injuries, minor damage to aircraft. Temperature was in the mid 40'S the day of the incident making the ice strip used for a runway extremely rough, ice slush and standing water. The ice strip used as a runway, is also used for vehicle traffic resulting in deep ruts in the ice. I am awaiting maintenance records, a letter has been sent to the pilot requesting records. An amended report will be completed as soon as the records and information is received.
