From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On April 1, 1999 delta airlines flight 1137 enroute from new orleans to Salt Lake City was issued ILS rwy 16L approach instructions. Delta flight 1137 was at 7000' on an eastbound base leg for rwy 16L at Salt Lake City International Airport. Delta flight 1137 did not acknowledge the approach instructions. Previously, there was normal communications flight 1137. Numerous attempts by air traffic on different transmitters and frequencies were unsuccessful. In the mean time the pilots of flight 1137 noticed their lost communications and changed to another radio. Communication was reestablished with flight 1137 and air traffic controller issued immediate instructions for flight 1137 to turn right and climb. Flight 1137 complied with the instructions and the aircraft was vectored for another approach and landed safely. Flight 1137 entered a 10400' minimum vectoring altitude (mva) area at 7000'. The aircraft's closest proximity to terrain was three miles laterally and 2500' vertically. The log book write up for flight 1137 (ship #407) stated "#1 radio failed twice could not transmit or receive. Volume levels were verified good. Radio came back on approx. 30 sec to 1 minute lat er. Radio faded out to zero and then faded back in". Delta maintenance corrective action: "replaced #1 VHF com transceiver ops normal". Captain antill written statement stated after crossing the localizer course rwy 16L he advised the fo to contact approach control. The fo was unable to contact approach on 31 transceiver, communications was reestablished using #2 transceiver. Captain antill stated a right climbing turn was initiated before communication was reestablished with approach control. This incident has been discussed with bob moore a representative of the poi for delta airlines certificate management (cmo). The cmo has been in contact with delta airline regarding this incident with emphasis on situation awareness. Dudley boone the FAA B-727 program manager assigned to delta airlines flew with captain antill on 4/16/99 from alt-iad-slc. According to inspector boone, captain antill conducted the flight in a professional manner with good CRM and situation awareness.
