From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Pilot indicated she thought she had a green gear down indication prior to take-off. Pilot selected gear up after take-off and noticed the amber gear in transit light was not lighted, the visable landing gear actuator clutch arm was not moving, and none of the other gear indicator lamps, red and green, were lighted. Pilot selected gear down, performed manual gear extension, and still no gear indicator lights. Pilot was told by control tower the gear appeared to be down. Pilot landed and during the roll-out the gear collapsed causing damage to the left brake cailiper, left gear door, right gear door, and tips of wooden prop. Recovery efforts caused minor damage to the underside of the engine cowling. Pilot's married name is pierce.
